How to make an appointment
To request an appointment:
- We operate a digital first GP lead Total Triage Appointment booking system via the NHS app and Accurx platform. This model ensures that appointments are booked with the most appropriate clinician, in the right time frame, first time. The Total Triage model provides improved access to, and more appropriate care. It allows us to maximise practice efficiency through better use of resources, directing patients to the most appropriate clinician first time.
- All requests for GP appointments must be submitted for triage. Our Triage platform opens at 08.00 weekday mornings and closes at 18.30. We would advise that any forms submitted after 4.30pm should be for routine purposes only. If you do submit a form for an urgent matter you may be advised to call 111 or attend an Urgent Care Centre or A&E.

Online appointments
We would ask that all patients that are able to please submit their appointment requests via either the NHS app or via our Practice website.Submitting your requests online is simple, quick and easy to access. You will be ask four mandatory questions and two optional questions, the online form will also give you the option to attach a photo if necessary.
Please include as much information as possible as this helps our triaging GP make the most informed decision regarding your ongoing care.

Patients who are unable to use the NHS App or access our website can book some appointments by calling reception on 01689 852204. Please note that this line is kept available for those patients who are unable to use the online system, due to not having access to this service.
Please bear in mind that our receptionists will need to ask you the same questions as is on our online form which will need to be completed fully. This is essential and ensures that our triaging GP has enough information to make an informed decision regarding your ongoing care.
In Person
Patients can visit reception who will help then complete the form.
If you need help when we are closed
If you need medical help now, use NHS 111 online or call 111.
NHS 111 online is for people aged 5 and over. Call 111 if you need help for a child under 5.
Call 999 in a medical or mental health emergency. This is when someone is seriously ill or injured and their life is at risk.

Home visits
If you are housebound and need an appointment, an online triage form will still need to be submitted. The GP will assess the best course of action which may be a visit from the GP or a third party ie Rapid Response or District Nurses
To request a home visit, this will need to be submitted as early as possible.
